Student and Caregiver Handbook

This manual provides useful insights into our program, as well as a thorough understanding of the policies and procedures that were created in the best interest of every child at our center. Their implementation ensures that we are in compliance with state licensure requirements and also assists in making our day to day operations run smoothly in the classroom. If you have questions or concerns about any information that is provided in this handbook, please do not hesitate to speak with our Director.

Enrollment Forms

The below forms need to be e-signed prior to starting at CLC. These forms include the following:

  • Parent/Student Guidelines
  • Discipline Policy
  • Tuition Policy
  • Health and Nutrition Policy
  • Photo and Video Release Form
  • Caregiver and Student Handbook Acknowledgement
  • Participation Agreement Waiver Release
  • Infectious Disease Agreement
  • Consent for Disclosure of Personally Identifiable Information and Immunization Information to Local and State Health Departments

If your student suffers from severe allergy related anaplylaxis, asthma, or seizures, please click on the corresponding link and complete the Action Plan. Return the completed form to the Program Director to discuss further.

Health Appraisal Form

The following form is required for all children prior to enrollment at CLC. This form is requested so that the school can work with the parent to meet the physical, intellectual, and emotional needs of the child as well as meet childcare licensing regulations. Please fill out the information requested in Section I. Section III may be certified by the transcription of information from the certificate of immunization. The remaining sections are to be completed by a doctor, nurse, and dentist.

Parent Satisfaction Survey

Parent and student satisfaction and success is of outmost importance to the staff of the Conductive Learning Center of North America. Following participation in at least one session at CLC, an email link will be sent for parents and caregivers to share feedback about participation in programming at the center, or you can submit it anytime using this link. Your feedback is very valuable to us and we appreciate your participation in this survey.

Scholarships

A scholarship fund is available for school year programs. Scholarships are based on income and may be applied for when enrolling in programming at Conductive Learning Center of North America. Please print and complete the application and send it to CLC along with a copy of your recent tax return. Your form can be mailed or dropped off at the office.Please note that you must also submit your session application in conjunction with your scholarship application. CLC also encourages families with financial needs to reach out to the various foundations that offer support to families of children with special needs.
